​Here's what you need to know:
Who can make a referral:

Participants, family members, support coordinators, or other service providers can submit referrals​​
Required documentation:

Current NDIS plan, relevant assessments or reports, and basic participant information.
Response timeframes:

We aim to acknowledge all referrals within 2 business days.
Funding requirements:

Our services primarily consist of NDIS funding, however we do accept other funding types depending on which services needed.
We understand that every person's situation is unique, and we're here to guide you through each step of the process.

​After a referral is received, here's what happens next:
Referral Review & Contact:

Referral review occurs within 2 business days

Our team will contact you to:

Request a copy of the NDIS plan

Collect any additional documentation needed

Confirm contact details

Right practitioner for you:

We'll match you with the most suitable practitioner based on:

Your specific needs
-
Location (within 30 minutes travel time)

Practitioner expertise and availability

Contract & Service Commencement:
-
You'll receive a service agreement to review and sign

Once everything is in place (signed agreement, NDIS plan, and PACE endorsement if required), we'll formally allocate your practitioner

Your practitioner will contact you within 5 business days to begin services
