Success Story:
Supporting a participant who had been involved with CCU and the Mental Health System as in involuntary consumer for majority of his adulthood to transition to a SIL accommodation which had been one of his biggest goals. The participant's support networks grew over the 1 year period of working together, and we were able to establish person centered planning to work towards new goals which included no longer being an involuntary consumer of the Mental Health Sector. This goal was also achieved whereby through capacity building and clear transitional planning, the participant was no longer on a treatment order, and instead voluntarily attended his own depot appointments with assistance from his care team members. This allowed the participant the opportunity to build on his own independent living skills and be the main driver in identifying what is important to him. As clear pathways, harm minimisation plans and transition plans were established, and there was consistency and stabilisation in the care team, Specialist Support Coordination was able to be phased out.
